Job summary

24 Hour Home Care in El Segundo is seeking a Social Intake Coordinator to manage Regional Center Social Assessment referrals from intake through completion. You will coordinate outreach, scheduling, assessments, and documentation in Salesforce, serving as the primary contact for individuals and families.

You will collaborate with Regional Center and internal teams to ensure timely assessments and accurate fee processing for billing.

Qualifications

• Bachelor’s degree required.
• Minimum 3 years of experience in person-centered practices, developing care plans, or related service-delivery environments preferred.
• Experience supporting individuals with intellectual and developmental disabilities or in social recreation or related services preferred.
• Bilingual Spanish preferred.

Responsibilities

• Manage Social Assessment referrals from intake through completion and submission, aiming for three assessments per day.
• Coordinate and schedule assessment appointments, serving as the primary contact for individuals and families.
• Conduct structured Social Assessments gathering social, developmental, behavioral, and environmental information while building rapport.
• Maintain accurate assessment documentation and Salesforce records with quality and turnaround.
• Partner with Regional Center and internal teams to address workflow needs and ensure fees are processed for billing.
• Support assessment and workflow needs across territories, adapting to priorities and assisting other areas as needed.
