Job summary

24 Hour Home Care is seeking a Social Intake Coordinator in El Segundo on a hybrid schedule, coming into the office 1x per month. The role centers on managing Regional Center Social Assessment referrals from intake to completion, coordinating outreach, scheduling, and documentation in Salesforce.

You will work with Regional Center and internal teams to ensure timely assessments and accurate billing, with a focus on high-quality customer service and de-escalation when needed.

Qualifications

• Minimum 3 years in person-centered practices or related service-delivery environment.
• Experience supporting individuals with intellectual/developmental disabilities or related services preferred.
• Strong communication and organizational skills.

Responsibilities

• Manage Social Assessment referrals from intake through completion and submission, conducting timely outreach and follow-up to complete an average of three assessments per day.
• Coordinate and schedule assessment appointments, serving as the primary point of contact for individuals and families throughout the assessment process.
• Conduct structured Social Assessments and gather required information while building rapport and providing excellent customer service.
• Complete and maintain accurate assessment documentation and Salesforce records, meeting quality, accuracy, and turnaround expectations.
• Partner with Regional Center and internal teams to address workflow needs and ensure completed assessments are tracked and ready for fee processing.
• Support assessment and workflow needs across assigned territories, adapting to changing priorities and assisting other service areas as needed to support team goals.
