Qualifications

• Active Florida RN license in good standing.
• Minimum 1 year of home health experience.
• Experience with Start of Care (SOC) and OASIS documentation.
• Experience with KanTime EMR system preferred.
• Excellent clinical assessment and critical thinking skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation and valid driver’s license.

Responsibilities

• Perform Start of Care (SOC) visits and comprehensive assessments.
• Develop individualized care plans and monitor patient progress.
• Complete accurate clinical documentation including OASIS.
• Coordinate with physicians, discharge planners, and interdisciplinary teams.
• Educate patients and families on plan of care, medications, and safety.
