Job summary

Matrix Home Health Care is seeking an experienced Registered Nurse (RN) to perform Start of Care visits in the Riviera Beach/Jupiter, FL area. The role focuses on initiating patient care, completing comprehensive assessments, and ensuring accurate OASIS documentation for home health patients.

You will deliver skilled nursing services, develop personalized care plans with patients and families, and collaborate with physicians, discharge planners, and the care team to achieve optimal outcomes.

Qualifications

• Active Florida RN license in good standing.
• Minimum 1 year of home health experience.
• Strong experience with Start of Care (SOC) and OASIS documentation.
• Experience with KanTime EMR system preferred.
• Excellent clinical assessment and critical thinking skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation and valid driver’s license.

Responsibilities

• Perform SOC visits and comprehensive assessments.
• Develop individualized care plans and monitor progress.
• Document OASIS and clinical notes accurately.
• Coordinate with physicians and discharge planners.
• Educate patients and families on plan of care and safety.
• Collaborate with interdisciplinary team for patient outcomes.
