Job summary

Matrix Home Care is seeking an experienced Registered Nurse (RN) to perform Start of Care visits in Riviera Beach, FL. This per diem role involves initiating patient care, completing assessments, and ensuring accurate OASIS documentation for home health patients.

You will develop and monitor individualized care plans, educate patients and families, and coordinate with physicians and the care team to ensure quality outcomes.

Qualifications

• Active Florida RN license in good standing.
• Minimum 1 year of home health experience.
• Experience with Start of Care (SOC) and OASIS documentation.
• Experience with KanTime EMR system preferred.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently with a flexible schedule.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide home-based care to patients with illnesses, injuries, and chronic conditions.
• Perform Start of Care (SOC) visits and develop individualized care plans.
• Examine patients, complete clinical assessments, and report changes to physicians.
• Complete accurate and timely clinical documentation, including OASIS and orders.
• Coordinate care with physicians, discharge planners, and interdisciplinary teams.
• Oversee and collaborate with LPNs, CNAs, and home health aides.
• Monitor patient progress and adjust care plans as needed.
• Discharge patients from services when goals are met.
