Matrix Home Health Care in Florida seeks experienced Registered Nurses (RNs) to perform Start of Care visits for home health patients in the Boynton/Lake Worth area. You will initiate patient care, conduct comprehensive assessments, and ensure accurate OASIS documentation for home health patients.

You will deliver skilled nursing services, develop and monitor care plans with patients and families, and coordinate care with physicians and interdisciplinary teams to achieve desired outcomes.

Qualificações

• Active Florida RN license in good standing.
• Minimum 1 year home health experience.
• Strong experience with SOC and OASIS documentation.
• Experience with KanTime EMR system preferred.
• Excellent clinical assessment and critical thinking skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation and valid driver’s license.

Responsabilidades

• Provide home-based care to patients with illnesses, injuries, and chronic conditions.
• Perform Start of Care (SOC) visits and develop individualized care plans.
• Examine patients, complete clinical assessments, document progress, and report changes to physicians.
• Complete accurate clinical documentation including OASIS, evaluations, physician orders, and care coordination notes.
• Coordinate care with physicians, discharge planners, and interdisciplinary teams.
• Oversee and collaborate with LPNs, CNAs, and home health aides.
