Job summary

Matrix Home Health Care is actively seeking experienced Registered Nurses (RNs) to perform Start of Care visits in the Vero/Sebastian, FL area. You will initiate patient care, complete comprehensive assessments, and ensure accurate OASIS documentation for home health patients.

The role requires an active Florida RN license and at least 1 year of home health experience, with opportunities to transition to full-time with benefits.

Qualifications

• Active Florida RN license in good standing.
• Minimum 1 year of home health experience.
• Experience with Start of Care (SOC) and OASIS documentation.
• KanTime EMR system experience preferred.

Responsibilities

• Perform Start of Care (SOC) visits and comprehensive assessments.
• Complete accurate OASIS documentation and care coordination notes.
• Develop, implement, and monitor individualized patient care plans.
• Educate patients and families on plan of care, medications, therapies, and safety.
• Coordinate care with physicians, discharge planners, and interdisciplinary teams.
