Qualifications
- Active Florida LPN license in good standing.
- Minimum 1 year of home health experience.
- Strong experience with OASIS documentation.
- Experience with KanTime EMR system preferred.
- Excellent clinical assessment and critical thinking skills.
Responsibilities
- Provide home-based nursing visits in Riviera Beach, FL.
- Examine patients, complete clinical assessments, document progress and report changes.
- Complete accurate OASIS documentation and care coordination notes.
- Develop, implement, and monitor patient care plans with patients and families.
- Deliver skilled nursing services and case management for assigned patients.
- Educate patients on plan of care, medications, and safety.
