Job summary

Alexander Youth Network seeks a SOAR Case Manager to join the Youth Focus Program at My Sister Susan’s House (MSSH). You will coordinate housing-related case management and help residents pursue education, employment, and independent living goals.

You will assess SOAR eligibility, gather documentation, and submit disability-based benefit applications while ensuring access to Medicaid and other public supports as needed.

Qualifications

• Must be at least 21 years old.
• Bachelor’s degree from four-year college or university; or one to two years’ experience and/or training; or equivalent combination of education and experience.
• Valid North Carolina Driver’s License.

Responsibilities

• Meet with housing residents to complete day-to-day case management at the MSSH, HEARTH TLP and HEARTH Beyond programs.
• Assist young adults in applying for mainstream benefits and support goals toward education, employment and independent living.
