Park Center Inc. in Nashville, TN is seeking a SOAR Outreach Specialist to provide program-specific street outreach to individuals experiencing mental illness and housing crises, screening and applying them for SSI/SSDI benefits, and coordinating hospital in-reach to ensure access to services.

You will engage clients, gather information, complete required documentation in HMIS/OAT, work with SSA offices, and coordinate with social work staff.

Qualificações

• Must have a Bachelor's degree in a related field.
• Minimum 2 years experience serving individuals facing homelessness or mental illness.
• Must be eager to learn and committed to the mission.

Responsabilidades

• Conduct street outreach to assist clients with housing, income, and services.
• Provide SOAR process support and coordinate with SSA to pursue disability benefits.
• Complete documentation in Park Center systems (HMIS, OAT, EMR).
• Engage with inpatient hospital clients and link to SOAR services.
• Collaborate with social work staff and treat physicians as needed.
