Job summary

The Primary Care Nurse Practitioner will join a growing medical group that provides primary care in post-acute and long-term care settings. You will be the main provider at skilled nursing facilities, delivering on-site primary and preventive care to residents where they live.

This on-site role involves managing acute changes in condition, coordinating with nursing staff and collaborating physicians, and leading care planning with residents and families.

Qualifications

• Active, unrestricted Nurse Practitioner license in Ohio.
• NP board certification (adult, family or geriatric) and an active DEA registration.
• 1+ years of experience as a practicing Nurse Practitioner.
• Clinical experience in long-term care, a nursing home or a skilled nursing facility.
• Comfortable working in an EMR and as part of an interdisciplinary team.
• Able to work on-site 4 days a week and live within about an hour of the assigned facility.
• Authorized to work in the U.S. without visa sponsorship.

Responsibilities

• Provide primary and preventive care to residents of skilled nursing facilities
• Manage acute changes in condition, chronic conditions and advanced illness
• Lead advance care planning conversations with residents and families
• Prescribe medications as allowed by Ohio law
• Order diagnostic tests and treatments and follow up on the results
• Work with nursing staff, interdisciplinary teams and collaborating primary care physicians
• Keep accurate, timely documentation in the EMR
