The NP will manage acute changes, plan advanced care, prescribe medications per OH law, and collaborate with a primary care physician to ensure comprehensive, patient-centered care. OH licensure, board certification, and DEA license are required.

Qualificações

• Current, unrestricted NP license in OH.
• Board certification is required.
• DEA licensure is required.
• Experience in long-term care is preferred.

Responsabilidades

• Provide primary and preventive care for long term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per OH law.
• Order, review and follow up on treatments and diagnostics.
• Communicate with nursing home staff regularly about changes in condition and care plan.
• Collaborate with the primary care physician.
