Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to ensure excellent patient care.

You will supervise Physical Therapist Assistants, re-evaluate progress, adjust plans of care, and complete timely documentation including evaluations and daily progress notes.

Qualifications

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Supervising Physical Therapist Assistants.
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

• Providing excellent care for our patients.
• Providing excellent customer service to our patients and family members.
• Collaborate with facility staff to ensure excellent care for your patients.
• Treating patients, educating them on their injury, progress and probable outcomes.
• Re-evaluating patient progress, revising plans of care and resetting goals.
