Job summary

Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team in SNF settings. You will evaluate and treat patients following an individualized plan of care, while collaborating with facility staff to ensure excellent patient outcomes.

The role includes supervising Physical Therapist Assistants, conducting re-evaluations, updating plans of care, and maintaining timely documentation of evaluations, progress notes, and discharges.

Qualifications

• Evaluating and treating patients according to individualized care plans.
• Ability to educate patients on injuries, progress and outcomes.
• Collaborating with facility staff to ensure high-quality care.
• Supervising Physical Therapist Assistants and coordinating care.

Responsibilities

• Evaluating new patients and developing plans of care with measurable goals.
• Treating patients and addressing concerns while educating them on progress and outcomes.
• Supervising PTAs and guiding their activities.
• Re-evaluating progress and revising plans of care as needed.
• Completing timely and accurate documentation (evaluations, re-evaluations, discharges, daily progress notes).
