Enhance Rehabilitation in the United States is seeking a Physical Therapist to join our team in the SNF setting, evaluating and treating patients following an individualized plan of care.

You will collaborate with facility staff, supervise Physical Therapist Assistants, and ensure timely documentation of evaluations, progress notes, and discharges, while communicating progress to patients and families.

Qualifikationen

• Qualified Physical Therapists with SNF experience preferred.
• Ability to evaluate and treat patients per individualized plan of care.
• Experience supervising PTAs.
• Strong documentation skills for evaluations and progress notes.

Aufgaben

• Providing excellent care for our patients
• Provide excellent customer service to patients and family members
• Collaborate with facility staff to ensure excellent care for patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations, discharges and daily progress notes.
