Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our growing team in a SNF setting. You will evaluate and treat patients following an individualized plan of care and work closely with facility staff to ensure high-quality care.

Responsibilities include evaluating new patients, developing plans of care, setting measurable goals, educating patients about progress and outcomes, and supervising Physical Therapist Assistants with accurate documentation.

Qualifications

• Provide excellent, patient-focused care.
• Deliver excellent customer service to patients and families.
• Collaborate with facility staff to ensure high-quality care.
• Evaluate new patients and develop plans of care with measurable goals.
• Treat patients, address concerns, and explain progress and outcomes.
• Supervise Physical Therapist Assistants.
• Complete timely documentation including evaluations, re-evaluations, discharges and progress notes.
