Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in a SNF setting. As a Physical Therapist, you will evaluate and treat patients following an individualized plan of care, delivering compassionate, evidence-based therapy to improve function.

You will collaborate with facility staff, educate patients and families about progress and outcomes, supervise therapy assistants, and complete timely documentation to ensure high-quality care.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
