Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in a SNF setting. As a Physical Therapist you will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure high-quality care and positive outcomes.

You will supervise Physical Therapist Assistants and complete timely documentation, including evaluations, re-evaluations, discharges and daily progress notes, while focusing on excellent patient service.

Qualifications

• Licensed Physical Therapist in good standing.
• Experience in SNF or long-term care preferred.
• Ability to develop individualized treatment plans.
• Excellent communication with patients and staff.

Responsibilities
