Job summary

The Vineyards Healthcare Center in Livermore, CA seeks a Physical Therapist to deliver an individualized therapy program, assess progress, and collaborate with the care team to maximize patient outcomes.

Qualifications

• Doctoral degree in Physical Therapy.
• California license to practice Physical Therapy is required.
• One year of experience as a physical therapy aide is preferred.

Responsibilities

• Provide individualized physical therapy program including assessment, planning, treatment and education to maximize functional goals.
• Direct patient care, evaluate functional needs and outcomes, and consult with other specialists.
• Design and implement plans of care based on assessments and involve significant others when appropriate.
• Conduct therapy within defined standards of care and supervise support staff to achieve goals.
• Maintain documentation, prescriptions and signatures, and participate in ongoing professional development.
