Job summary

The Vineyards Healthcare Center in Livermore, CA is seeking a dedicated Registered Nurse to join our skilled nursing facility team. You will assess residents, plan individualized care, administer medications, and collaborate with interdisciplinary teams to ensure high-quality care.

We welcome fresh or newly licensed RNs and prefer experienced SNF nurses. A CA RN license, active BLS and CPR certifications, and flexibility to work weekends/holidays are required.

Qualifications

• Freshly licensed RNs welcome.
• Experienced RNs preferred.
• Experience in a skilled nursing facility/long-term care a plus.
• CA RN license required.
• Active BLS through the American Heart Association.
• Active CPR certification.
• Interest in working with the geriatric population.
• Ability to work in a fast-paced environment and manage multiple priorities.
• Willingness to work flexible hours, including weekends and holidays.

Responsibilities

• Conduct thorough assessments of residents' health status and needs.
• Develop and implement individualized care plans based on assessments.

• Monitor residents' progress and adjust care plans as needed.
• Provide education to residents and families about health conditions and care options.
• Collaborate with interdisciplinary teams to ensure optimal care.

We are seeking a dedicated and compassionate Registered Nurse to join our team at a skilled nursing facility.

RESPONSIBILITIES:

• Conduct thorough assessments of residents' health status and needs.
• Develop and implement individualized care plans based on assessments.
• Administer medications and treatments as prescribed by physicians.
• Monitor residents' progress and make necessary adjustments to care plans.
• Provide education to residents and families about health conditions and care options.
• Collaborate effectively with interdisciplinary teams to ensure optimal care.
• Document all aspects of patient care accurately and timely in electronic records.

QUALIFICATIONS:

• Fresh or newly licensed RNs are welcome
• Experienced RN is preferred
• Experience in a skilled nursing facility/long-term care/senior care is a plus
• A valid and active California license as a Registered Nurse is required
• Must have an active BLS through the American Heart Association
• Must have an active CPR Certification
