Evolve Therapy Services in Waterbury, CT seeks a Speech-Language Pathologist for an in-house Skilled Nursing Facility. Roles include direct therapy, data-driven treatment planning, and family/staff education to ensure carryover of goals. In-house position supports CMS-compliant documentation and FEES/MBS oversight.

The candidate will hold a Master’s in Speech-Language Pathology, CCC, and an active SLP license with ongoing continuing education. Full Time, Part Time, or PRN options available.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC certification is required.
• Active SLP license in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards.
• Evaluate data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Educate patient, family, and staff for carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening to meet needs timely and with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
