Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house skilled nursing facility in Pahokee, FL. The role is available as part-time or PRN, focusing on planning, directing, and delivering speech pathology services in accordance with federal, state, and local standards.

Responsibilities include evaluating patients, developing treatment plans, documenting thoroughly in NetHealth EMR, and coordinating FEES/MBS protocols.

Qualifications

• Master's degree in Speech-Language Pathology required.
• CCC required (Certificate of Clinical Competency).
• Active SLP license in the practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsabilités

• Provide quality speech-language pathology services and initiate referrals.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals (short- and long-term).
• Instruct patients, families, and staff to ensure carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols with site coordination.
• Screen residents to identify needs and ensure timely, high-quality care.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Part Time or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
