Job summary

Evolve Therapy Services in Warren, PA is seeking a Speech Language Pathologist to join our in-house Skilled Nursing Facility program. The role focuses on planning, organizing, developing, and directing Speech Language Pathology services to ensure high-quality resident care.

The SLP will provide therapy, develop treatment plans with goals, educate patients and staff, and ensure CMS-compliant documentation in NetHealth. A CCC and active state license are required, with ongoing CEU support.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Active SLP license in state(s) of practice required.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover into daily activities.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocol implementation and coordination of site programs.
• Participate in screening protocols to establish resident needs with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.


• To evaluate and obtain data necessary for treatment planning and implementation


• To develop a treatment plan that includes a target list, short-term, and long-term goals


• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities


• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems


• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs


• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology


• Certificate of Clinical Competency


• MUST have active SLP license in state(s) of practice.


• Ongoing continuing education and professional development to maintain current licensure and certification
