Job summary

Evolve Therapy Services is seeking a compassionate Speech Language Pathologist to provide services in an in-house Skilled Nursing Facility in Warren, PA. This PRN opportunity supports resident care through evaluation, treatment planning, and responsive therapy services.

Qualified candidates hold a Master's in Speech Language Pathology with CCC, and an active SLP license in the applicable state. Ongoing CEUs and strong collaboration with the care team are essential for success.

Qualifications

• Master's degree in Speech-Language Pathology required.
• CCC certification required.
• Active SLP license in the state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services in accordance with standards and CMS expectations.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Instruct patients, families, and staff on speech-language procedures for carryover.
• Complete compliant documentation in NetHealth EMR.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.


• To evaluate and obtain data necessary for treatment planning and implementation


• To develop a treatment plan that includes a target list, short-term, and long-term goals


• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities


• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems


• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs


• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology


• Certificate of Clinical Competency


• MUST have active SLP license in state(s) of practice.


• Ongoing continuing education and professional development to maintain current licensure and certification
