Job summary

Evolve Therapy Services in Massena, NY is seeking a Speech Language Pathologist to provide in-house therapy at a partner Skilled Nursing Facility. You will plan, develop, and direct SLP services in compliance with federal, state, and local standards, under the direction of site administration.

Responsibilities include evaluating data for treatment plans, creating goals, instructing residents and staff, and ensuring CMS-compliant documentation in NetHealth EMR.

Qualifications

• Masters in Speech Language Pathology required.
• CCC certification and active state licensure required.
• Ongoing continuing education to maintain licensure/certification.
• Experience with CMS documentation and facility protocols is preferred.

Responsibilities

• Plan, organize, develop, and direct SLP services per standards and regulations.
• Provide quality speech therapy and initiate referrals when appropriate.
• Evaluate data for treatment planning and implement goals.
• Develop and monitor treatment plans with short- and long-term goals.
• Instruct patient, family, and staff for carry-over into daily activities.
• Ensure CMS-compliant documentation in NetHealth EMR and oversee FEES/MBS protocols.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
