Job summary

Evolve Therapy Services in New Haven, CT is seeking a compassionate Speech Language Pathologist to join our in-house Skilled Nursing Facility team. This PRN position focuses on delivering high-quality therapy services and coordinating care within CMS standards.

You will evaluate residents, develop treatment plans with short- and long-term goals, document in NetHealth EMR, and participate in FEES/MBS protocols with site staff.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC (Certificate of Clinical Competency) required or eligible.
• Active SLP license in state(s) of practice required.
• Commitment to ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate appropriate referrals.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Educate patients, families, and staff on speech-language procedures.
• Document CMS-compliant, medically necessary notes in NetHealth EMR.
• Oversee FEES/MBS protocols with site program coordination.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
