Continuum Therapy Partners, LLC is seeking a Speech-Language Pathologist for a PRN daytime role at Montgomery Nursing & Rehab Center in Hillsboro, IL. The position emphasizes continuity of care and collaboration with a leadership team of therapists who understand delivering high-quality care on your terms.

The role offers flexibility with a PRN schedule and opportunities to contribute to patient outcomes while working alongside our established clinical standards.

Formación

• Master's Degree in Speech-Language Pathology required.
• Current, unencumbered state SLP license; CCC-SLP required.
• 1 year of previous experience as a Speech-Language Pathologist preferred.

Responsabilidades

• Evaluates and develops individualized treatment plans for speech, language, and swallowing disorders.
• Documents patient progress and adjusts treatment plans based on clinical outcomes.
• Educates patients and families on communication strategies and swallowing safety.

Speech-Language Pathologist Requirements & Responsibilities

• Master's Degree in Speech-Language Pathology required


• Current, unencumbered state SLP License; ASHA Certificate of Clinical Competence (CCC-SLP) required


• 1 year of previous experience as a Speech-Language Pathologist preferred


• Evaluates and develops individualized treatment plans for speech, language, and swallowing disorders


• Documents patient progress and adjusts treatment plans based on clinical outcomes


• Educates patients and families on communication strategies and swallowing safety
