Job summary

Flagstar Rehab is seeking a Speech Language Pathologist (SLP) for a Skilled Nursing Facility in New York. You will perform speech and swallow evaluations, develop care plans, and provide therapy to SNF patients in a dynamic setting.

We offer flexible schedules (FT/PT/Per Diem), weekly pay, and benefits options. Strong NYS SLP license with CCC, CPR, and EMR competency are required; CFY not accepted. Join our team to impact patient care and collaboration.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Active NYS SLP license and CCC.
• CPR certification.
• Ability to work in SNF and manage swallow evaluations.

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients.
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture.
• Provide quality therapy services to individuals in an SNF setting with related conditions.
• Monitor and document patient progress with EMR.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Speech Language Pathologist Job Duties

• Provide efficient speech or swallow evaluations and/or screens of patients


• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture


• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.

• Not accepting Clinical Fellows (CFY) at this time.
