Job summary

Flagstar Rehab seeks a Speech Language Pathologist (SLP) for a skilled nursing facility in New York. The role focuses on evaluations, plan of care, and quality therapy for patients with conditions such as TBI, CVA, and Parkinson’s. Flexible schedules include full-time, part-time, per diem, weekends, and evenings.

Qualifications

• Must hold NYS SLP license and CCC.
• Current CPR certificate and EMR competence required.
• US employment in a nursing facility per state and federal law.
• Malpractice/liability insurance may be applicable.

Responsibilities

• Provide efficient speech and swallow evaluations and screens.
• Develop plan of care and manage meal textures.
• Deliver therapy services in SNF for diverse diagnoses.
• Monitor progress and document using EMR software.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Speech Language Pathologist Job Duties:

• Provide efficient speech or swallow evaluations and/or screens of patients
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.

• Not accepting Clinical Fellows (CFY) at this time.
• Current NYS SLP License and Registration
• Clinical Competence Certification (CCC)
