Job summary

Flagstar Rehab is seeking a Speech Language Pathologist (SLP) to provide therapy in a Skilled Nursing Facility in New York. You will perform evaluations, develop plans of care, and deliver therapy to patients with diverse conditions, including TBI, stroke, Parkinson’s, and MS.

We offer flexible schedules (FT/PT/Per Diem), weekly pay, and benefits such as paid orientation, sick leave, commuter benefits, and LLC support for independent work. NYS SLP license and CCC required; CFY not accepted.

Qualifications

• Not accepting Clinical Fellows (CFY) at this time.
• Current NYS SLP License and Registration
• Clinical Competence Certification (CCC)
• CPR Certificate
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, MS, Cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress with the use of EMR software.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Speech Language Pathologist Job Duties:
