Job summary

Flagstar Rehab offers a Speech Language Pathologist (SLP) position in a Skilled Nursing Facility. The role includes efficient evaluations, care plan development, and delivering therapy for patients with diverse conditions such as TBI, Stroke, Parkinson’s, and MS, supported by EMR documentation.

The position supports flexible scheduling (Full-Time, Part-Time, Per Diem, Weekends, Days/Evenings) with weekly direct deposit, paid orientation, and optional insurance options.

Qualifications

• Current NYS SLP License and Registration
• CCC Certification (Clinical Competence Certification)
• CPR Certificate
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Speech Language Pathologist Job Duties

Speech Language Pathologist Requirements

• Not accepting Clinical Fellows (CFY) at this time.
• Current NYS SLP License and Registration
• Clinical Competence Certification (CCC)
