Job summary

Flagstar Rehab is seeking a Speech Language Pathologist (SLP) for a Skilled Nursing Facility role. We connect passionate SLPs with opportunities that fit their goals, offering flexible schedules and streamlined onboarding to get you into the right position quickly.

Qualifications

• Current NYS SLP license and registration.
• CCC certification required.
• CPR certification required.
• EMR competence required.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Not accepting Clinical Fellows (CFY) at this time.

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients.
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture.
• Provide therapy to SNF patients with conditions including stroke, TBI, Parkinson’s, MS, cancer, and orthopedic issues.

Speech Language Pathologist Job Duties:

• Provide efficient speech or swallow evaluations and/or screens of patients
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.
