Job summary

Horizon Home Health Care in Ohio is seeking a licensed Speech-Language Pathologist to provide high-quality speech therapy in patients' homes across Montgomery County and surrounding areas. This part-time/PRN role offers autonomy, flexible scheduling, and one-on-one patient care with support from a clinical team.

You will assess and treat speech, language, cognitive, voice, and swallowing disorders, develop individualized goals, educate families, document care, and ensure compliance with home

Qualifications

• Current Ohio SLP license.
• CPR and/or BLS certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Home health experience is preferred, but not required.

Responsibilities

• Evaluate and treat speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on patient goals.
• Educate patients and caregivers on treatment strategies and home programs.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians, nurses, therapists and other disciplines of the healthcare team.
• Maintain compliance with state and federal home health regulations.
