Job summary

Alternate Solutions Health Network, LLC is seeking a Speech Therapist to provide home-based skilled therapy across designated Ohio territories. You’ll work with an interdisciplinary team to deliver compassionate, patient- and family-centered care in the patient’s preferred environment.

The role offers a guaranteed base salary plus an uncapped bonus structure, mileage reimbursement, and a supportive team with scheduling and admissions assistance. A valid license and driver’s license are required.

Qualifications

• Current license as a Speech Therapist in the employing state.
• Minimum one year of ST experience in acute care; home care preferred.
• Valid driver's license and auto insurance in the driver's name.

Responsibilities

• Develop individualized speech therapy plans of care and adjust as needed.
• Educate patients and families to support safe, effective home-based therapy.
• Monitor patient progress and report changes to the attending physician.
• Counsel family on disease/condition management across therapy phases.
• Complete clinical documentation following agency standards and Medicare guidelines.
• Follow all agency policies and change schedule as required.
• Participate in in-service trainings and mandatory meetings.

You will educate patients and their family members and ensures the safety of the patient.MAJOR AREAS OF RESPONSIBILITY:Plan of Care: Develop an individualized ST plan of care and makes adjustments as needs change. Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.Minimize Patient Risk: Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decision.Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy. Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy.Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.Operations: Attend in-service training and mandatory agency meetings.HARD & SOFT SKILLS:Compassionate communicator with a positive attitude.Patience is a virtue when working with patients, families, physicians, and coworkers.Attention to detail is critical, as is being observant and following directions.REQUIREMENTS:Speech Therapist with a current license in the state of employment.Minimum one-year experience as a ST in an acute care setting.
