Job summary

Southwoods Health is seeking a compassionate Speech-Language Pathologist (SLP) to join our Home Health team on a per diem basis, serving patients in their homes throughout the Mahoning Valley. You will conduct evaluations, deliver therapy, and coordinate with physicians to tailor plans of care.

The role requires Ohio licensure as a Speech Therapist, current BLS, and the ability to work independently in home settings. Per diem scheduling offers flexibility and patient-first focus.

Qualifications

• Education: Graduate of an accredited Speech-Language Pathology program.
• Licensure: Active Ohio State Licensure as a Speech Therapist.
• Certification: Current American Heart Association (AHA) Basic Life Support (BLS) certification.
• Experience: Prior experience in Home Health therapy preferred.
• Key Skills: Strong interpersonal communication, clinical problem-solving, and the ability to work independently in a home-care setting.

Responsibilities

• Patient-Centered Care: Conduct comprehensive evaluations and administer medically prescribed treatments to help patients overcome communication, cognitive, or swallowing disorders.
• Clinical Collaboration: Partner with physicians and multidisciplinary teams to develop and adapt treatment plans based on patient progress and medical necessity.
• Family Education: Empower patients and their families by providing essential education on recovery processes, physician protocols, and home exercise programs.
• Documentation Excellence: Maintain accurate clinical records in accordance with state, federal, and Southwoods standards, ensuring timely reporting of patient outcomes.
• Mentorship: Provide supervision for students and support personnel in alignment with Ohio licensure requirements.
