Job summary

Sweetwater Care in California is seeking a licensed LVN to provide nursing care within the scope of practice and according to physician orders, center policies, and state/federal guidelines.

The role requires prior nursing management experience and long term care exposure, with familiarity in Point Click Care charting. The LVN will participate in rounds and educate patients and families as part of ongoing care.

Qualifications

• Currently licensed as LVN in California.
• One-year prior nursing management experience preferred.
• Long term care experience.
• Point Click Care charting experience.

Responsibilities

• Provides nursing care within scope of practice in line with standards and policies.
• Demonstrates timely medication administration per facility policy.
• Delivers treatments timely and per facility policy.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
