Qualifications

• Provide comprehensive medical care to SNF patients with acute and chronic conditions.
• Lead interdisciplinary care teams and coordinate transitions of care.
• Maintain licensure, CME, and ethical practice standards.
• Communicate clearly with patients, families, and care teams.

Responsibilities

• Perform initial admissions evaluations and develop treatment plans.
• Supervise discharge planning and coordinate transfers to reduce readmissions.
• Manage medications with nursing staff and consulting providers.
• Ensure timely, accurate clinical documentation and goals of care.
