Qualifications

• MD or DO licensed physician
• Maintains licensure and CME

Responsibilities

• Provide comprehensive medical care to SNF patients with acute and chronic conditions
• Perform initial admission evaluations and develop treatment plans
• Lead interdisciplinary care teams and coordinate transitions of care
• Educate patients and families on health conditions and care plans
• Complete required clinical documentation in a timely manner
• Collaborate with APPs, nursing staff, pharmacy, and other providers
