• Admissions & Care Planning: Serve as the primary contact for prospective residents and families; Manage referrals, pre-admission assessments, and the full admissions process; Collaborate with others to create and update resident care plans; Participate in interdisciplinary team meetings to review progress and plan discharges; Coordinate insurance notifications, appeals, and communication; Complete and submit MDS assessments according to regulatory requirements.
