ClarkLindsey is a welcoming senior living community in Urbana, IL, seeking a Nurse Coordinator (RN) to lead resident care and clinical coordination for Assisted Living & Memory Care.

You will assess residents, update care plans, and support nurses, CNAs, and resident assistants while promoting person-centered care and regulatory compliance. A full-time schedule includes on-call rotation and benefits.

Qualifications

• Illinois RN license required.
• Geriatric nursing experience with dementia care.
• Proficiency with PointClick Care and MS Office (Word/Excel/Teams/SharePoint).
• Full-time role with on-call rotation.

Responsibilities

• Assess residents and develop individualized care plans.
• Monitor resident health status and communicate changes to physicians and families.
• Provide leadership and supervision to nursing staff and onboarding.
• Administer medications and ensure regulatory compliance.
