Job summary

Pivotal Health Care, through Linden Woods Village in Gladstone, MO, seeks an Admissions Coordinator to assess referrals, plan admissions, and advocate for residents’ wellbeing. The role centers on pre-admission discussions and discharge planning within a continuing care retirement community.

Ideal candidates will be licensed RNs or LPNs, preferably with 2 years of geriatric experience, and possess strong communication and organizational skills. A valid driver's license is required.

Qualifications

• Graduate from an accredited school and licensed RN or LPN in state.
• If LPN, enrollment in RN program within 6 months and continue progress.
• Maintain licensure and CEU requirements.
• Prefer 2 years of geriatric experience.
• Must possess a valid driver’s license.

Responsibilities

• Pre-admission assessment of referrals and discharge planning.
• Communicate effectively with residents, families, physicians, and care teams.
• Attend facility meetings and in-service training.
• Coordinate admissions and discharge planning and care plan meetings.
• Report monthly discharges to the Ombudsman and maintain referral tracker.
• Support resident-centered/directed care and advocacy.
