  • Minimum 5 years of elder care coordination experience, preferably with private or high-net-worth clients.
  • Strong understanding of healthcare systems and long-term care options.
  • Exceptional organizational, interpersonal, and communication skills.
  • Ability to manage confidential situations with empathy and discretion.
  • Hybrid role with both virtual and in-person support; travel may be required.

Responsibilities

  • Coordinate appointments and communicate between providers and family.
  • Arrange and oversee in-home care, including 24/7 support if needed.
  • Source and evaluate assisted living, memory care, skilled nursing, and hospice care.
  • Assist with medical coordination, including chronic illness management and wellness planning.
  • Coordinate care, therapies, rehabilitation, and educational resources for family members.
  • Provide guidance and logistical support for serious illnesses.
  • Source and evaluate medical professionals and insurance coverage.
  • Advocate for family members within Medicare, private insurance, and long-term care policies.
  • Review medical bills for accurate coding and payments.
  • Leverage life insurance and long-term care benefits to optimize options.
  • Stay current on healthcare regulations and elder care best practices.

Job description

The Family Office Care Coordinator is a trusted advisor to multigenerational family. This role ensures personalized and dignified care across all stages of aging and serves as a liaison between the family, healthcare providers, and care institutions, ensuring seamless integration of services and optimal use of available resources. The preference of the family member determines what coordination is needed to meet their needs.

Key Responsibilities:
Care Management
  • Coordinates appointments and may attend at the request of the family member, ensuring accurate communication between providers and family.
  • Arrange and oversee in-home care, including 24/7 support if needed.
  • Source and evaluate top tier assisted living, memory care, skilled nursing facilities and hospice care.
  • Assist with medical coordination, including chronic illness management and wellness planning.
  • Source and support family members by coordinating care, therapies, rehabilitation and educational resources.
  • Provide guidance and logistical support when a family member is diagnosed with a serious illness.
  • Source and evaluate medical professionals.
  • Source and evaluate insurance coverage.
  • Advocate for family members within the healthcare system, including Medicare, private insurance, and long-term care policies.
  • Review the medical bills to ensure accurate coding and insurance information before out-of-pocket payments are made.
  • Leverage life insurance and long-term care benefits to optimize care options.
  • Stay current on healthcare regulations, elder care innovations, and best practices.

Qualifications:

  • Hybrid role virtual and in-person support.
  • Flexible schedule with availability based on needs of the family.
  • May require local and out of state travel.
