Job summary

HonorHealth Deer Valley Medical Center in Tempe, AZ seeks an experienced Case Manager to coordinate care across inpatient and outpatient settings. You will work with patients, families, caregivers, and a multidisciplinary team to ensure smooth transitions, optimal outcomes, and high-quality, cost-effective care.

Responsibilities include discharge planning, collaboration with hospitalists, specialists, and PCP, and advocacy for patients within regulatory standards and payer requirements.

Qualifications

• 1 year as a Licensed Social Worker, and/or successful completion of health related field placement in Master’s level Social Work Program - Required
• 2 years in case management - Preferred
• 3 years as a Licensed Social Worker - Preferred
• Other Experience in management of behavioral health patients - Preferred

Responsibilities

• Accounts for an assigned caseload and collaborates with patients, caregivers, healthcare providers, and external partners to ensure coordinated care and a smooth transition of care.
• Facilitates discharge planning by communicating medical information and plan of care between hospitalists, specialists, and PCP; explains financial obligations and discharge details to patient and family.
• Supports development and maintenance of Case Management metrics and manages caseload to ensure quality, cost-effective care.
• Acts as a patient advocate and coordinates with physicians, families, and payers to ensure adherence to standards of care and applicable guidelines.
