Job summary

HonorHealth is seeking a Case Manager within the Tempe Medical Center to coordinate inpatient and outpatient care, ensuring smooth transitions and high-quality outcomes. The role emphasizes collaboration with hospitalists, PCPs, and external partners to optimize discharge planning and post-acute support.

The ideal candidate holds a Master’s Degree in Social Work (Required) and an active SW license (LSW/LMSW/LCSW).

Qualifications

• Master's Degree in Social Work required.
• 1 year as a Licensed Social Worker required.
• 2 years in case management preferred.
• 3 years as a Licensed Social Worker preferred.
• Licensure: LSW/LMSW/LCSW required.

Responsibilities

• Coordinates care with patients, caregivers, and providers to ensure smooth transitions.
• Discharge planning and post-acute care coordination for appropriate levels of care.
• Collaborates with health care team to develop and monitor case management metrics.
• Advocates for patients and uses resources to support care plans and community resources.
• Performs other related duties as assigned.
