Job summary

Qualifications

• MSW required.
• 1 year as a Licensed Social Worker or health-related field placement in Master's level program required.
• 2 years in case management preferred.
• 3 years as a Licensed Social Worker preferred.

Responsibilities

• Collaborates with patients and caregivers early in inpatient or outpatient episodes to prepare for discharge, including post-acute interventions and advance directives.
• Facilitates smooth transition from acute care to appropriate level of care and communicates with hospitalists, specialists, PCP and payers for discharge planning and authorization.
• Promotes collaboration among the health care team and maintains caseload while contributing to case management metrics.
• Acts as patient advocate, guiding communication and ensuring adherence to standards of care and applicable laws/regulations.
