Job summary

Breakthrough seeks a compassionate Transitional Housing Program Assistant to support women and men in our Adult Service Network. The role helps residents move from crisis to stability by providing intake, case-management coordination, and access to services.

Based at the Women’s Center in Chicago, you’ll work up to 25 hours per week with varied shifts, collaborating with staff and partner agencies to promote self-sufficiency and dignity for each guest.

Responsibilities

• Meet with guests in Transitional Housing Program in partnership with case manager to facilitate transition from crisis to stability
• Conduct intake and follow preventative procedures with all individuals entering the facility during the shift
• Provide supportive services to residents as needed to support their goals toward self-sufficiency
• Enforce all rules and regulations of Breakthrough’s Homeless Intervention Program
• Coordinate, communicate with other staff, guests and partnering agencies in a positive, effective, and professional manner
• Enter participant information into the Salesforce/HMIS database
