Society St. Vincent de Paul Alameda County seeks an Assistant Case Manager to support shelter guests experiencing homelessness.

You will work under the Case Manager, assisting with needs assessment, resource navigation, housing applications, and ongoing documentation to help guests achieve housing stability. The role emphasizes compassionate, confidential service, collaboration with shelter staff, and thorough follow-up to connect guests with medical, behavioral health, employment, and housing

Qualifications

• Experience in social services, case management, housing services, shelter services, outreach, behavioral health, or related field preferred.
• Ability to follow established procedures and seek direction from the Case Manager when situations exceed the scope of the position.
• Experience with HMIS or a similar case management database preferred.
• Commitment to serving individuals experiencing homelessness with dignity, compassion, and respect.
• Strong follow-through and attention to detail.
• Willingness to become a Mandated Reporter, with training provided as required.
• Ability to work effectively with individuals from diverse backgrounds and maintain confidentiality.
• High school diploma or equivalent required; higher education preferred.

Responsabilités

• Support the Case Manager in providing client-centered services to shelter guests.
• Assist guests in identifying immediate needs, barriers, goals, and available resources.
• Provide one-on-one support, advocacy, and resource navigation under the Case Manager.
• Assist guests in accessing food, transportation, identification documents, benefits, health care, behavioral health services, employment resources, and other services.
• Help guests prepare for appointments, complete applications, gather documentation, and follow up on referrals.
• Maintain regular communication and follow-up with assigned guests.
• Assist Case Manager with housing-navigation activities and housing applications.
• Maintain accurate, timely, and confidential client documentation and HMIS entries.
• Assist with tracking referrals, service linkages, and housing outcomes.
• Collaborate with shelter staff and community partners to support positive outcomes for guests.
• Participate in staff meetings, case consultations, trainings, and professional development.
