Job summary

Qualifications

Responsibilities

• Conduct intake within 24 hours of admission.
• Document psychosocial, vocational/educational, and housing assessments.
• Develop initial service plans and conduct reviews for caseloads.
• Provide referrals for in-house and community services.
• Ensure clients attend meals and maintain nutritional intake.
• Assist in discharge/transfer processes and housing interviews.
