Qualifikationen

• High School/GED Diploma or equivalent preferred.
• 1–2 years of experience with homeless adults, substance use or mental health populations.
• Ability to work in a team and coordinate with partners; bilingual skills are a plus.

• Conduct intake and psycho-social assessments for assigned clients.
• Provide case management and counseling to help clients move toward housing and independence.
• Develop and monitor Individual Living Plans with short- and long-term objectives.
• Input client information into the CARES system and maintain documentation.
• Assist in group sessions and coordinate with community housing providers and service programs.
