Qualifikationen

Aufgaben

• Conduct intake process with new clients within 24 hours of admission; explain resident rights and grievance procedures.
• Conduct psychosocial, vocational/educational, and housing assessments and document findings.
• Provide timely documentation of supportive social services and client progress toward permanent housing.
• Prepare housing applications and assist clients with housing processes.
• Monitor client progression in behavior and appearance.
• Facilitate group and individual counseling, workshops, and activities.
• Maintain client charts in compliance with DHS/OTDA and SUS standards.
• Provide referrals for medical, mental health, and social services.
• Ensure clients attend meals and maintain nutrition.
